What is CORE?

Community Outreach Recovery and Engagement (CORE) is a free, community-based mental health support service for people across the Bradford District living with Severe Mental Illness (SMI), or those experiencing significant emotional distress.

How We Help

Our experienced support workers offer up to 12 one-to-one sessions tailored around your needs. We’ll work with you to improve your wellbeing and help you connect with services that can support your wider needs.

Who Is This For?

CORE is for adults (16+) in Bradford District who:

  • Have a diagnosed SMI, or
  • Are experiencing significant emotional distress impacting their daily life

Please note: This service is not for people who are:

  • Already receiving secondary mental health support
  • An immediate risk to themselves or others
  • Struggling with alcohol or substance misuse that prevents engagement

How It Works

1. Referral

A professional can refer you, such as a GP, mental health practitioner, or someone from your local Community Mental Health Team. Referrals are linked to your GP practice.

3. Initial Appointment

Within 10 working days, you’ll meet your support worker to talk about your goals and the kind of support that suits you.

2. First Contact

A CORE support worker will get in touch within 5 working days of receiving your referral.

4. Ongoing Sessions

You’ll receive up to 12 sessions. We’ll review your progress at session 6 and again at session 12 to help plan next steps.

How to Access CORE

Speak to your GP, mental health worker, or local health professional to get started. Referrals must be made by a professional and are based on your GP practice.

Frequently Asked Questions

Who can refer to CORE?

Any professional working with you can make a referral, including GPs, mental health practitioners, or someone from the Community Mental Health Team.

Do I need a diagnosis?

No. You may still qualify if you’re experiencing emotional distress that impacts your quality of life.

What if I’m already under secondary mental health care?

CORE is not designed for those already supported by secondary care teams.

Can I refer myself?

No, referrals must be made by a professional. If you’re unsure who to speak to, your GP is a good place to start.

How long will I get support for?

You’ll receive up to 12 sessions. We review support halfway through and at the end to ensure you’re getting what you need.

How long does it take to receive support after I am referred?

We aim to contact all referrals promptly to assess needs and provide initial support. The timeline may vary depending on your specific requirements and the services involved.

Where does support take place?

Support is flexible, in person at a local venue, over the phone, or online. We’ll work around what works best for you.
