Make a Referral

Referrals to the Wellbeing Hub can be initiated by both professionals and individuals. To begin the process, please contact us via email at wellbeinghub@incic.co.uk. or call: 01274 665598

Getting Started:

Upon receiving your referral, a member of our team will contact you to assess your needs and facilitate connections with the appropriate resources. We are dedicated to guiding you through each step, ensuring that the support you receive is perfectly tailored to your unique circumstances.

Referral Critaria:

  • All ages, but children (under 18) must be accompanied by an adult
  • People registered with GP practices in the Bradford District and Craven footprint
  • If unregistered, living within the local authority boundaries of City of Bradford Metropolitan District Council and Craven District Council.
  • People with mild/ moderate physical/ mental health needs

You are welcome to walk in during our opening hours, Monday to Friday, 9am to 5pm. However, booking an appointment in advance ensures that a staff member will be available to assist you.

Is there a cost for the services provided at the Wellbeing Hub?

All services offered at the Wellbeing Hub are free of charge to support community well-being.

Can I bring a family member or friend for support during my visit?

Absolutely. You are welcome to bring someone along to help support you during your visit.

What should I bring with me for my first appointment?

If possible, bring any relevant documents related to the issues you would like help with (e.g., benefit letters, identification, or previous service records). This helps us better understand and address your needs.

How long does it take to receive support after I am referred?

We aim to contact all referrals promptly to assess needs and provide initial support. The timeline may vary depending on your specific requirements and the services involved.

Are the services at the Wellbeing Hub accessible for people with disabilities?

What happens if I need help outside the Hub’s opening hours?

For urgent issues, we recommend reaching out to emergency services or local helplines. If it’s a non-urgent matter, please email us, and we will get back to you as soon as possible during business hours.
